Terms of Service Policy

Eligibility

Services are intended for patients legally eligible to receive care in Florida or through appropriate authorized pathways. Patients must provide accurate identity, contact, medical, and payment information.


Medical Evaluation and Provider Judgment

All treatments are subject to provider evaluation and approval. DWLC may decline, delay, modify, or discontinue treatment when clinically appropriate.


No Guarantee of Treatment

Submitting a form, making payment, scheduling an appointment, or communicating with DWLC does not guarantee approval, medication, prescription, refill, procedure, result, or outcome.


Patient Accuracy

Patients certify that information submitted is accurate and complete. Misrepresentation, omission, or inaccurate medical information may result in denial, delay, cancellation, or discontinuation of services.


Medical Weight Loss Programs

GLP-1, GLP-1 & GIP, semaglutide, tirzepatide, phentermine, appetite suppressant, hormone-related wellness programs, testosterone-related therapy, IV hydration or infusion therapy, vitamin injections, and other medical weight loss or wellness programs are subject to provider review and medical appropriateness. Dosing is individualized and may change based on response, safety, availability, and provider judgment.


Prescription Medication Use

Patients agree to:

Use medications only as directed. Not share medications. Follow storage instructions. Report adverse effects. Contact DWLC with concerns. Seek emergency care when appropriate.


Med Spa and Aesthetic Services

Aesthetic services are elective. Results vary. Additional sessions may be recommended. Patients must follow pre-treatment and post-treatment instructions. DWLC may refuse treatment when medically or aesthetically inappropriate.


Laboratory and Wellness Services

Wellness bloodwork, injections, and related services are provided according to clinic protocols. Lab results may require review by an appropriate licensed healthcare provider. Patients should not rely on generalized educational information as a diagnosis.


Telehealth

Telehealth may be used when appropriate. Patients understand telehealth has limitations and that in-person examination, laboratory testing, or referral may be required.


Payments

Patients authorize payment for selected services or programs. Flexible payment options may be available at checkout through third-party payment providers, but approval is not guaranteed.


Refunds

Refunds are governed by DWLC’s Refund, Return & Cancellation Policy.
