Doctors’ Weight Loss & Med Spa (“DWLC,” “we,” “our,” or “the clinic”) is committed to providing respectful, professional, patient-centered care. This Patient Bill of Rights applies to patients receiving services through DWLC in clinic, by phone, electronically, or through telehealth where applicable.

Respect, Dignity, and Privacy
Patients have the right to be treated with courtesy and respect, with appreciation of individual dignity, personal values, and privacy.Non-Discrimination
Patients have the right to receive care without discrimination based on race, color, religion, national origin, sex, age, disability, marital status, sexual orientation, gender identity, or other protected status.Information About Providers
Patients have the right to know who is providing care, the role of each provider or staff member involved, and who is responsible for their treatment plan.Medical Information and Treatment Explanation
Patients have the right to receive understandable information about their condition, proposed treatment, medication options, expected benefits, possible risks, alternatives, and the consequences of refusing treatment.Participation in Care Decisions
Patients have the right to participate in decisions about treatment, ask questions, and accept or decline treatment, subject to applicable law and provider judgment.Medical Evaluation Required
All DWLC medical treatments and prescription programs are subject to provider evaluation and approval. Payment, form completion, or scheduling does not guarantee approval, prescription, medication dispensing, or treatment outcome.Confidentiality and Medical Records
Patients have the right to confidentiality of medical records and communications in accordance with applicable federal and Florida law. Patients may request access to medical records according to clinic procedures and applicable law.Financial Information
Patients have the right to receive information about program pricing, fees, payment terms, refund rules, and billing policies before treatment begins.Questions, Concerns, and Complaints
Patients have the right to ask questions and raise concerns regarding care, billing, privacy, or clinic policies. Concerns may be directed to DWLC at 727-412-8208.Emergency Care
DWLC does not provide emergency medical services. Patients with chest pain, severe allergic reaction, difficulty breathing, fainting, severe abdominal pain, suicidal thoughts, or other urgent symptoms should call 911 or seek emergency care immediately.Patient Responsibilities
Patients are responsible for:
Providing accurate and complete medical history, medication lists, allergies, pregnancy status, prior reactions, and relevant health information. Updating DWLC regarding changes in health status, medications, address, phone number, email, and shipping information. Using medications only as prescribed and not sharing medications with others. Reading and following pharmacy labels, dosing instructions, storage instructions, and safety warnings. Reporting side effects, concerns, or adverse reactions promptly.
Keeping appointments or notifying the clinic if unable to attend. Completing required forms, evaluations, consents, and payment steps before treatment processing. Treating staff, providers, and other patients respectfully.
